PRC-PALM-SPC-5.51-pro3-frm4: INSTRUCTIONS FOR THE COLLECTION OF SEMEN SPECIMEN FROM POST-VASECTOMY PATIENTS

1. Put your name, birth date, and date and time of collection on the container.
2. Specimen should be collected directly into container by masturbation on the morning it is to be examined.
3. Collection into a condom is unacceptable.
4. It is necessary to abstain from sexual contact 3 days prior to collection. Specimen must be delivered to laboratory within 1 hour of collection.
5. Deliver specimen to Henry Ford Laboratory [list labs here].

Note: All SEMEN ANALYSIS, other than POST VASECTOMY CHECK, is performed at the Reproductive Medicine Clinic at Troy IVF, 2825 Livernois Rd. Troy Michigan 48083. All testing is performed by appointment only. Call (248) 637-4050 to schedule an appointment. .

NOTE: Remember to bring written laboratory orders with your specimen in order to properly process your tests.
